888 Improving the Documentation of Skin Closure and the Plan for Its Removal for Trauma & Orthopaedic Patients

Abstract Aim Surgical patients require closure of wounds with skin clips or sutures. Removal at an appropriate time can prevent complications, namely infection. Our goal is to ensure clear documentation type and plan for removal. The Royal College Surgeons states that “details technique” “detailed post-operative care instructions” are required. Method Retrospective data collections were undertaken in January September 2022 trauma Altnagelvin Hospital. Data was gathered from Operation Notes. first collection included 30 patients, the second 24 patients. Two main interventions implemented. these presentation initial findings a departmental audit meeting purpose education. amend operative proformas include different types its removal prescribed post-op as “delete appropriate” options. Results Following our interventions, correct documented 100% Plan 84.6% overall between both written typed notes. Typed notes had compared only 66.67% In collection, 55% adequate plans on so amended being introduced have resulted improvement. Conclusions Accurate essential patient safety simple result drastic improvement quality. Pre-typed superior should be used where available appropriate.
